ON 07/14/2021, Licensing Program Analyst (LPA) arrived unannounced to conduct a subsequent complaint inspection. LPA introduced self, stated the purpose of the visit and requested to meet with the Administrator. LPA met with Administrator, Pamela Hamilton. During this visit, LPA conducted staff interviews. Based on record review and interviews, the above allegation is found to be UNSUBSTANTIATED. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. No deficiencies cited during this inspection. An exit interview was conducted. As a COVID-19 precautionary measure, a copy of this report will be provided to Administrator via email and an electronic read receipt confirms receiving this document.
