What the inspector wrote

On 7/20/23 at 11:04 AM, Licensing Program Analyst (LPA) Malia Thao arrived unannounced to conduct an initial 10-day complaint inspection. LPA explained reason for inspection and was granted entry. LPA met with Licensee/Administrator (LIC) Jonathan Johnson and Facility Manager (FM) Nicole Morehead. LPA made observations, reviewed records, and conducted interviews. Based on interviews and record review, LPA found that although the facility initially denied R1 from returning to the facility upon notification that R1 was ready for hospital discharge, the facility accepted the return of R1 the same day. Therefore, the above allegation is unsubstantiated. The allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation occurred. Exit interview conducted. A copy of this report was given to Licensee, whose signature confirms receipt of this report.
