Based on LPA review of staff criminal record clearance and associations, the licensee did not comply with the section cited above as Staff S1 was not associated to the facility prior to working at the facility which poses an immediate health, safety or personal rights risk to persons in care.

Based on LPAs review of staff files, the licensee did not comply with the section cited above in 1 of 3 staff files reviewed, the staff file had no required documentation including ID information, Health, screening, TB, documentation of training which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on review of resident records and staff statements, the licensee did not comply with the section cited above as they were unable to produce copies of recent medical assessment for newly admitted residents which poses/posed a potential health, safety or personal rights risk to persons in care.
