On 10/14/2025 Licensing Program Analyst (LPA) Shakaricka Hughes conducted a visit to the facility. The purpose of the visit was to conduct a case management visit. LPA met with the facility designated administrator/licensee Veniana Banuve. The current census is 6. During today's visit, it was determined that the facility administrator did not meet the minimum qualifications as required under Title 22 RCFE regulation 87405(a) Administrator Qualifications and Duties. Additionally, observations and staff interviews indicated that the facility was operating with insufficient staffing to meet the needs of all residents in care. The lack of adequate staffing presents a potential risk to residents health and safety as it may limit staff's ability to provide timely assistance with activities for daily living, and supervision of residents in care. The administrator Veniana acknowledged the need to ensure that sufficient qualified staff are present in the facility at all times to meet the residents care needs. The administrator agrees to increase staffing levels in the facility, including being present in the facility as the administrator for a sufficient number of hours to permit adequate attention to the management and administration of the facility. An exit interview was conducted with Veniana, and a copy of the LIC 809, LIC 809-D, and Appeals Rights were provided to the facility.
