On 1/3/23 at 9:45AM, Licensing Program Analyst (LPA) Chris Hopkins conducted an unannounced facility visit in regards to a complaint investigation with the above allegations. LPA Hopkins met with Administrator H.C Samala and explained the purpose of today's visit. Regarding the allegation of Facility is prohibiting visitors, the Department found the following; based on interview and observation, it was determined that Resident 1 (R1) did not want to have visitors. Administrator did inform family that R1 did not want to see them, facetime, or talk on the phone. The department has investigated the above allegation and has determined that it is unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. Exit Interview was conducted, and a copy of this report was provided to the facility.
