On 7/1/25, Licensing Program Analyst (LPA) Arian Golbakhsh conducted and unannounced Case Management visit to conduct a brief Health and Safety visit as well as to follow up on a Hospice Waiver request from the facility. LPA was welcomed by, identified themselves to, and discussed the purpose of the visit to Caregiver Danielle Thurman. Assistant Administrator Meredith Pritchard-Malo arrived later during the visit. LPA briefly toured the facility, conducted file review and obtained copies of records, and spoke with staff and residents. LPA did not observe any immediate health and/or safety violations and after speaking with residents did not receive any complaints about their wellbeing. LPA observed residents participating in activities and staff preparing meals. LPA provided consultation with Assistant Administrator Pritchard-Malo regarding hospice waivers and exceptions. No deficiencies were cited during the visit. An exit interview was conducted with Assistant Administrator Pritchard-Malo to whom a copy of this report and the Licensee/Appeal Rights (LIC 9058) were provided. Their signature below confirms receipt of these documents.
