What the inspector wrote

Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted an announced pre-licensing inspection to assess the facility’s compliance with Title 22 Code of Regulations and the Health and Safety Code. LPA met with Administrator Angelita Galamgam. The facility has applied for a change in facility type from an Adult Residential Facility (ARF) to a Residential Facility for the Elderly (RCFE) to serve four (4) ambulatory residents age 60 and older. The facility was previously licensed to serve four (4) ambulatory adults age 18 and older. Fire clearance for this application was approved on March 20, 2025. During today’s inspection, LPA observed that all indoor and outdoor passageways were free of obstructions. No pools or bodies of water were present. According to Administrator Galamgam, there were no firearms, ammunition, or other dangerous weapons on the premises. Toxins, cleaning supplies, and chemicals were locked and inaccessible. LPA toured all rooms designated as resident bedrooms and observed that each room was furnished with a bed, chair, nightstand, dresser, and adequate lighting. The Administrator confirmed that residents are provided with sufficient hygiene products for personal use. The facility was equipped with multiple operational smoke and carbon monoxide detectors that met statutory requirements. (continue at LIC809C) (continue from LIC809) LPA observed three (3) fire extinguishers that appeared to be in working order. The facility maintained a two-day supply of perishable food and a seven-day supply of nonperishable food. Medications were stored in a locked cabinet and were properly labeled and maintained in accordance with label instructions. Resident and staff records were secured and stored in compliance with regulations. The pre-licensing inspection and Component III were completed on today’s date. The facility is ready for licensure pending management approval. This application reflects a change in facility type from ARF to RCFE for four (4) ambulatory residents. At the time of the visit, all residents in care were ambulatory. An exit interview was conducted with Administrator Galamgam. A copy of this report and the Licensee/Appeal Rights were provided to the Administrator. The Administrator’s signature on this form confirms receipt of these documents.
