On 1/12/24 at 1:20pm Licensing Program Analyst (LPA) Ashley Akinleye arrived at the facility to conduct a complaint investigation. LPA was met by Patricia Sorto and granted access to the facility. LPA explained to licensee reason for visit. Per the complaintant, the facility was operating out of ratio. LPA conducted multiple interviews and toured the facility with the licensee. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated. Exit interview conducted with licensee Patricia Sorto. Appeal rights and a Notice of Site Visit were provided to the licensee to post for 30 consecutive days.
