What the inspector wrote

On 07/29/2025 at 9:30 AM, Licensing Program Analysts (LPAs) K. Nguyen and P. Manalo arrived unannounced to conduct 1-Year Annual Required inspection. LPAs was greeted by care staff, Emilia Malimba. Care staff informed Administrator (AD) Alejandria Bautista (AD). LPA spoke with AD and explained the purpose of the visit. LPAs conducted the inspection process, later AD arrived. LPAs toured facility including but not limited to front entrance, screening station, hand washing stations, bedrooms, common areas, kitchen, and backyard. Smoke Detector and Carbon Monoxide is in operating condition. Fire drill was conducted on 06/10/2025. Fire extinguisher last check on 08/15/2024. Emergency plan was last posted on 7/04/2025. Facility has a current liability insurance from 2025-2026. LPAs reviewed 3 staff record and 3 out of 3 have TB on file, also associated to the facility. LPAs reviewed 6 residents files. THE FOLLOWING DEFICIENCIES WERE OBSERVED: At 10:17 AM, LPAs observed Purell Disinfectant Spray in the living room counter. At 10:39 AM, LPAs observed cleaning items in the bathtub. The above deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations. Failure to correct deficiencies by POC date may result in additional Civil Penalties.
