What the inspector wrote

On 3/12/2026 at 12:00 PM, Licensing Program Analyst (LPA) Y. Brown arrived unannounced to conduct a Case Management visit. LPA met with Alex Juntilla, Administrator and explained the purpose of the visit. While LPA Y. Brown was conducting a complaint investigation (15-AS-20260309124645) on 3/12/2026 during file review LPA discovered: 1. S1 was not associated to the facility. The deficiency was observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties. Exit interview conducted with Alex and a copy of this report and appeal rights provided.

Based on observation, the licensee did not comply with the section cited above in that the staff did not grant ombudsmen access to the facility which poses an immediate personal rights risk to persons in care.

    (e) All individuals subject to a criminal record review... shall prior to working.. in a licensed facility: (3) Request a transfer of a criminal record clearance...This requirement was not met as evidence by: Based on record review the Licensee did not comply with the section cited above in having S1 associated to the facility, which poses an immediate health and safety risk to person in care.
