What the inspector wrote

On 12/9/2021 at 12:00PM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conduct a case management visit. LPA met with Executive Director, Dolly Rizvi. During the course of investigation for complaint (#15-AS-20210812150406), the following deficiency was observed. Interview with staff indicated that staff was unaware of R1's open wound on the leg despite ADL (Activities of Daily Living) care was provided to R1 with 2-person assist. The deficiency was observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct deficiency may result in civil penalties. Exit interview conducted. A copy of this report and appeal rights provided.
