On 08/11/2025 at 11:30 AM, Licensing Program Analyst (LPA) David Doidge arrived unannounced to conduct the quarterly inspection of the facility to ensure that the requirements of the 05/05/2025 stipulation are being followed. LPA met with Marie Milare, Licensee and explained the purpose of the visit. During this visit, the LPA observed: (1) stipulation posted in a conspicuous place, (2) criminal record clearances or exemptions for all staff are maintained, (3) current personnel records of each employee are maintained, (4) an accurate, complete, and current weekly staff schedule, covering all shifts is available, (5) payroll records were available, (6) medications and any other toxic products, poisons, detergents, cleaning compounds and other hazardous objects and materials are properly stored and kept in a safe and locked place, and (7) records of training are being kept. Licensee is complying with all aspects of stipulation. No citations issued during visit. Exit interview conducted and a copy of this report provided.
