What the inspector wrote

On 03/16/22 at 11:20AM, Licensing Program Analyst (LPA) Catherine Lin conducted an unannounced complaint visit, met with Resident Service Director (RSD) Joann Nisperos and explained the purpose of the visit. LPA toured the Salem Woods building with RSD to inspect the elevator. LPA observed RSD tried to press the button and use FOB key to activate elevator in the garage which did not work. LPA interviewed 2 residents at Salem Woods, they both stated that they were not able to use elevator from garage to upper levels in the last several months. The maintenance director stated that he started getting quotations from different companies lately. Based on LPA’s observations and interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegation was found to be SUBSTANTIATED. Deficiency is cited per Title 22 California Code of Regulations and listed on LIC9099D. Failure to submit proof of correction (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
