What the inspector wrote

Allegation: Resident's call button is in disrepair. Based on interview and records review, staff admitted that the alert system that is showing on the computer is in disrepair due to down wifi signal, although the wifi is down the pager system is active all the time, staff still receiving alerts from the residents pendants. Allegation: Facility has inadequate staffing. Based on interview and records review, based on staff roster and work schedules it was revealed that there are 3-4 staff for morning shift, PM shift have 3-4 staff were scheduled at 2 staff at night time. Residents stated when they needed assistance, staff were always there to help. Although the allegation may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did occur, therefore the allegation is UNSUBSTANTIATED. Exit interview conducted with Anthony Garcia. A copy of this report provided. Pendant summary reports indicated that about 22 cases (6/26/2022-7/3/2022) where response times were 30 minutes long. LPA reviewed a few of those cases and most cases were residents activating pendant. Based on LPA’s observations and interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegation was found to be SUBSTANTIATED. Deficiency is cited per Title 22 California Code of Regulations and listed on LIC9099D. Failure to submit proof of correction (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties. Exit interview conducted , report and appeal rights provided.
