What the inspector wrote

On 7/24/25 at 2:50 PM Licensing Program Analyst (LPA) Greg Clark arrived unannounced to conduct a case management visit. LPA met with Anthony Garcia, Administrator(ADM) and explained the purpose of the visit. LPA requested that ADM send LPA all invoices related bug bed treatment for the year 2025 by 7/30/25. Exit interview conducted, a copy of this report provided

Facilities shall not involuntarily transfer or evict residents for grounds other than those specifically enumerated under state law or regulations, and shall comply with enumerated eviction and relocation protections for residents. For purposes of this paragraph, “involuntary” means a transfer, discharge, or eviction that is initiated by the licensee, not by the resident.
