On 03/25/2025 at 1:10 PM, Licensing Program Analyst (LPA) Ardalan Gharachorloo arrived unannounced to conduct a case management visit to provide explain the process of applying for a hospice waiver. LPA met with the Administrator, Ohnmar Shwe and explained the purpose of the visit. During the visit, LPA explained to the administrator the process of applying for hospice waiver as well as the total care exception. LPA asked if the administrator is applying for a hospice waiver or a total care exception. The administrator stated that she is applying for a hospice waiver and provided portion of the documents to the LPA. The remaining documents will be submitted to the LPA at a later date. Exit interview conducted and a copy of this report provided.
