What the inspector wrote

On 07/18/2025 at 10:50 AM, Licensing Program Analysts (LPAs) L. Alexander and K. Nguyen arrived unannounced to conduct Pre-Licensing inspection. LPAs met with Lead Staff, Humphrey Kasweka and explained the purpose of the visit. Humphrey called the Administrator, Leea Burns to inform. Administrator, Leea Burns arrived shortly. The facility currently has three (3) residents. Administrator Certificate # 7006161740 expires 05/15/2027 . LPAs toured facility with Humphrey including but not limited to three (3) bedrooms, two (2) bathrooms, kitchen, common areas and backyard. Bedrooms and living rooms were equipped with the proper furniture. Bathrooms were equipped with grab bars and non-skid mats. Linens and hygiene supplies were observed inside a cabinet. There is sufficient lighting throughout facility. Room temperature was maintained at 70 degrees F and hot water temperature was maintained at 110 degrees F. First-aid kit was observed to be complete. Smoke detectors and carbon monoxide were operational. Fire extinguisher was last serviced on 10/09/2024. LIC809-C Continued... THE FOLLOWING DEFICIENCIES WERE OBSERVED DURING VISIT: At 12:19pm unlocked Chemical in garage on table and in file cabinet, big bathroom, underneath bathroom cabinet of small bathroom, and on the kitchen counter (i.e., Clorox Multi-Surface Cleaner and Bleach, Clorox Mist Spray, Pine-Sol) At 12:20pm unlocked medication in file cabinet (Robitussin, Milk of Magnesia, and prescription Chlorhexidine…), and prescription thickener on the kitchen table from previous resident. At 12:23pm unlocked hammer tool in garage, drill tool in client dresser, nail, and pair of scissors located in kitchen on dish rack. At 12:40pm Collective garbage bags on the side of the facility including boxes, and two white doors. At 12:25pm urine odor smell in shared ADA bathroom At 2:30pm No RCFE Training on record for eight (8) Direct Support Provider (DSP) At 2:35pm Staff without TB clearance Prior to licensure, the following shall be corrected and faxed to CCL by 08/01/2025: Exit interview conducted and a copy of this report provided.
