What the inspector wrote

On 12/03/2025 at 11:00 AM, Licensing Program Analysts (LPA) P. Manalo announced to conduct pre licensing inspection. LPAs met with Licensee/Applicant, Thinn Aye, and Administrator, Thant Lynn, and explained the purpose of the visit. The facility has an approved fire clearance for six (6) bedridden. LPA toured facility with Licensee/applicant including but not limited to 6 bedrooms which all bedrooms will be occupied by the residents, 5 bathrooms, two kitchen area, two living space area, and backyard area. Bathrooms were equipped built in non-skid shower pan. Linens and hygiene supplies were observed inside a cabinet. There is sufficient lighting throughout facility. Room temperature was maintained at 61 degrees F and hot water temperature was maintained at 125.6 degrees F. First-aid kit was observed to be complete. Smoke detectors and carbon monoxide were operational. Fire extinguisher was last serviced on 02/06/2025. The following will need to be completed before recommending licensure to Centralized Application Bureau (CAB): At 11:00 AM, LPA observed that all the residents’ room does not have a chest of drawers. At 11:02 AM, LPA observed the RCFE Personal Rights were not posted. At 11:05 AM, LPA observed multiple drawers, cabinet space, shower area with debris, backyard fence leaning inward towards the facility's passageway, the attic area missing a roof panel, and the garage filled with debris such as window net, cubicle supplies, etc. Continue to LIC809-C... Continue from LIC809... At 11:10 AM, LPA observed hot water temperature measured at 125.6 degrees Fahrenheit. At 12:24 PM, LPA observed the liability insurance does not cover the total annual aggregate requirement. This facility is not yet licensed, and is subject to final approval by CAB. Additional requirements may still be required. Exit interview conducted and a copy of this report was provided to Licensee/applicant.
