What the inspector wrote

On 12/18/2025 at 9:00 AM, Licensing Program Analyst (LPA) P. Manalo announced to conduct pre licensing inspection. LPA met with Licensee/Applicant, Thinn Aye and explained the purpose of the visit. The facility has an approved fire clearance for six (6) bedridden. LPA toured facility with Licensee/applicant including but not limited to 6 bedrooms which all bedrooms will be occupied by the residents, 5 bathrooms, two kitchen area, two living space area, and backyard area. Bathrooms were equipped built in non-skid shower pans. Linens and hygiene supplies were observed inside a cabinet. There is sufficient lighting throughout facility. Room temperature was maintained at 68 degrees F and hot water temperature was maintained 112.2 and 110.6 degrees F. First-aid kit was observed to be complete. Smoke detectors and carbon monoxide were operational. Fire extinguisher was last serviced on 02/06/2025. The following corrections were noted during the visit: LPA observed that all the residents’ rooms have a chest of drawers. LPA observed the RCFE Personal Rights and the Rights of Resident Counsel posted on the wall. LPA observed multiple drawers, cabinet space, shower area without debris, the side fence in the backyard is fixed, the roof panel attached to the attic, and the items removed from the garage. Continue to LIC809-C... Continue from LIC809… LPA observed hot water temperature measured at 112.2- and 110.6-degrees Fahrenheit. LPA observed the liability insurance covers the total annual aggregate requirement. No issues noted during inspection. LPA observed that the facility is ready to be licensed. This report will be submitted to the Centralized Applications Bureau (CAB) and a final review of the application will be conducted. The facility is not yet licensed and is subject to final approval by CAB. Additional requirements may still be necessary. Exit interview conducted and a copy of this report was provided to the licensee.
