What the inspector wrote

On 04/29/2026 at 1:05 PM, Licensing Program Analyst (LPA) P. Manalo conducted an unannounced Case Management Visit to follow up on the updated facility sketch and Fire Safety Inspection Request (STD 850). LPA met with Licensee, Thinn Aye, and explained the purpose of the visit. On 04/01/2026, LPA received an email from Licensee requesting to change the staff bedroom to a different room. On 04/09/2026, LPA received the approved STD 850 from the fire department. During today's visit, LPA verified that the staff rooms have been changed consistent with the updated facility sketch submitted and approved by the fire department. No deficiencies cited. Exit interview conducted and a copy of this report provided.
