What the inspector wrote

On 04/29/2026 at 1:30 PM, Licensing Program Analyst (LPA) P. Manalo conducted an announced Case Management Visit. LPA met with Licensee, Thinn Aye and explained the purpose of the visit. While LPA was at the facility for another visit, LPA observed the following: At 1:22 PM, LPA observed unlocked medications in R1 and R2's room. Per physician report, R1 and R2 are not able to store their on medications. At 1:22 PM, LPA observed unlocked Miralax in the second kitchen. LPA and Licensee also discussed that residents that are transferred from the other sister facility will need to have updated and completed resident forms. A technical assistance was issued. The Facility was cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiencies by POC date may result in additional Civil Penalties. Exit interview conducted with Licensee. Appeal Rights and a copy of this report provided.
