What the inspector wrote

On 12/09/2025 at 9:55 AM, Licensing Program Analyst (LPA) P. Manalo arrived announced to conduct pre licensing inspection. LPAs met with Licensee/ Applicant, Mario Rodriguez, and Administrator, Olive Neri, and explained the purpose of the visit. This is pre-license for a change of ownership. LPA toured facility with Administrator and Licensees/ Applicants including but not limited to 4 residents' bedrooms, 2 bathrooms, one office space, kitchen, living room, and backyard area. Bathrooms were equipped with grab bars and non-skid shower pan. Linens and hygiene supplies were observed inside a cabinet. There is sufficient lighting throughout facility. The hot water temperature in a residents’ shared bathroom was measured at 114.1. First-aid kit was observed to be complete. Smoke detectors and carbon monoxide were operational. Fire extinguishers were observed to be full and last serviced on 02/20/2025. The facility has a working telephone that was verified during the visit. Prior to licensure, the following shall be corrected and faxed to CCLD by 12/15/2025: At 10:14 AM, LPA observed cart filled with debris and wheelchair in the backyard. Staff stated that they will schedule a bulk pick up for the items. At 10:19 AM, LPA observed 3 cartons of eggs in the kitchen pantry. Continue to LIC809-C... Continue from LIC809... At 10:20 AM, LPA observed an insect in the kitchen pantry. At 10:32 AM, LPA observed Lysol wipes unlocked in the office. At 10:36 AM, LPA observed a sectional couch blocking the passageway of the sliding door exit. At 10:38 AM, LPA observed glass cleaner and Lysol spray unlocked in the bathroom. At 10:39 AM, LPA observed a broken dresser handler in Room#4. At 10:40 AM, LPA observed the curtain in Room #4 in disrepair. At 10:51 AM, LPA observed Lysol wipes unlocked in the laundry room. At 11:00 AM, LPA observed mold/ mildew in both clients’ bathrooms. At 11:30 AM, LPA observed S2, S3, and S4 without a CPR certification. At 11:45 AM, LPA observed that R4 does not have a doctor’s order for the half bed rail. At 12:00 PM, LPA observed the residents' record not signed and dated. At 12:05 PM, LPA observed the emergency disaster plan needs to be updated with the correct information. At 12:10 PM, LPA did not observe the Resident Counsel Rights Poster This facility is not yet licensed, and is subject to final approval by CAB. Additional requirements may still be required. Exit interview conducted and a copy of this report was provided to Licensee/applicant.
