What the inspector wrote

On 12/29/2025 at 12:55 PM, Licensing Program Analyst (LPA) P. Manalo announced to conduct pre licensing inspection to verify the corrections have been made. LPA met with Licensee/Applicant, Mario Rodriguez, and explained the purpose of the visit. LPA toured facility with Administrator and Licensees/ Applicants including but not limited to 4 residents' bedrooms, 2 bathrooms, one office space, kitchen, living room, and backyard area. Bathrooms were equipped with grab bars and non-skid shower pans. Linens and hygiene supplies were observed inside a cabinet. There is sufficient lighting throughout facility. First-aid kit was observed to be complete. Smoke detectors and carbon monoxide were operational. Fire extinguishers were observed to be full and last serviced on 02/20/2025. The facility has a working telephone that was verified during the visit. The following corrections were noted during the visit: LPA observed the backyard with debris was cleared. LPA observed the pest control plan service agreement was started on 11/12/2025. LPA observed the sectional couch removed from the passageway in Room #4. LPA observed the dresser and curtain repaired in Room #4. Continue to LIC809-C... Continue from LIC809-C... LPA observed cleaning chemicals such as Lysol Wipes, glass cleaner, Lysol spray locked. LPA both clients’ bathrooms unsoiled. LPA observed S2, S3, and S4 with a CPR certification. LPA observed that R4’s doctor’s order for the half bed rail. LPA observed the residents' record completed. LPA observed the emergency disaster plan updated with the correct information. LPA observed the Resident Counsel Rights Poster. LPA observed that the facility is ready to be licensed. This report will be submitted to the Centralized Applications Bureau (CAB) and a final review of the application will be conducted. The facility is not yet licensed and is subject to final approval by CAB. Additional requirements may still be necessary. Exit interview conducted and a copy of this report was provided to the licensee.
