Pre-licensing visit·December 30, 2025

On 12/30/2025 at 9:55 AM, Licensing Program Analyst (LPA) P. Manalo arrived announced for pre-licensing visit. LPA met with Applicant, Edith Sarmiento, and Administrator Fatima Lacsamana and explained the purpose of the visit. LPA toured facility including but not limited to 6 bedrooms, one staff room, 2 full bathrooms, 2 half bathrooms, kitchen, and common area. Bedrooms and living rooms were equipped with the proper furniture. Bathrooms were equipped with grab bars and non-skid shower pans. Linens and hygiene supplies were observed inside a cabinet. There is sufficient lighting throughout facility. Room temperature was maintained at 63 degrees Fahrenheit. Hot water temperature was measured at 122.5 and 106.2 degrees Fahrenheit. First-aid kit was observed to be complete. Smoke detectors and carbon monoxide were operational. Fire extinguisher was last serviced on 05/30/2025. LPA reviewed 2 resident records and 2 staff records. LPA reviewed client’s P&I money log. LPA reviewed client’s medications. All records were observed to be complete and up to date. The following will need to be completed before recommending licensure to Centralized Application Bureau (CAB): Liability Insurance will need to cover the total amount for the general aggregate. Hot water temperature measured within range. – Completed during today’s visit. Hot water temperature was measured at 117.1 degrees Fahrenheit. Exit interview conducted and a copy of this report provided.
