Pre-licensing visit·March 3, 2026

On 03/03/2026 at 8:55 AM, Licensing Program Analyst (LPA) P. Manalo arrived announced for pre-licensing visit. LPA met with Applicant/ Administrator, Monique Tran and explained the purpose of the visit. LPA toured facility including but not limited to 6 bedrooms, one office room, 2 full bathrooms, 3 half bathrooms, kitchen, and common area. Bedrooms and living rooms were equipped with the proper furniture. Bathrooms were equipped with grab bars and non-skid shower pans. Linens and hygiene supplies were observed inside a cabinet. There is sufficient lighting throughout facility. Room temperature was maintained at 70 degrees Fahrenheit. Hot water temperature was measured at 106.4 and 115.6 degrees Fahrenheit. First-aid kit was observed to be complete. Fire alarm and carbon monoxide were operational. Fire extinguisher was last serviced on 05/30/2025. LPA reviewed 2 resident records and 3 staff records. LPA reviewed client’s P&I money log. LPA reviewed client’s medications. The following will need to be completed before recommending licensure to Centralized Application Bureau (CAB): S3 will need to be associated with the current facility. - Cleared during the visit. Exit interview conducted and a copy of this report provided.
