Pre-licensing visit·May 1, 2026

On 05/01/2026 at 12:35 PM, Licensing Program Analyst (LPA) P.Manalo arrived unannounced to conduct Pre-licensing Inspection. Upon arrival, LPA met with Applicant/ Licensee, Yolanda Uy and explained the purpose of the visit. The facility currently has no clients. LPA toured facility including but not limited to 5 bedrooms, 3 bathrooms, kitchen, common areas and backyard. Bedrooms and living rooms were equipped with the proper furniture. Bathrooms were equipped with grab bars and non slip mat . Linens and hygiene supplies were observed inside a cabinet. There is sufficient lighting throughout facility. Room temperature was maintained at 68 degrees Fahrenheit and hot water temperature was maintained at 113.9 degrees Fahrenheit. First-aid kit was observed to be completed. Smoke detectors and carbon monoxide were operational. Fire extinguisher was last serviced on 02/25/2026. The following will need to be completed before recommending licensure to Centralized Application Bureau (CAB): - Obtain a working telephone for the facility. -Purchase a new thermometer. This facility is not yet licensed, and is subject to final approval by CAB. Additional requirements may still be required. Exit interview conducted and a copy of this report was provided to Licensee/applicant.
