Pre-licensing visit·March 5, 2026

On 3/5/2026 at 9:30AM, Licensing Program Analyst (LPA) G. Luk conducted a Pre-licensing Inspection. LPA met with Licensee(s)/Applicant(s), Saul Cantolino and Rita Cantolino. The facility's fire clearance was approved for 5 non-ambulatory residents and 1 ambulatory resident. LPA toured facility including but not limited to resident's bedrooms, bathrooms, living room, kitchen, garage, and outdoor area. Hot water was measured at 110.9 degrees F in the hallway bathroom. LPA observed bathrooms have grab bars and non-skid mats. LPA observed lighting in all rooms. LPA observed facility has non-perishable and perishable food supplies available. Smoke and carbon monoxide detectors were observed in operating conditions. First aid kit was complete. Emergency disaster plan and infection control plan was complete. The following will need to be completed before recommending licensure to Centralized Application Bureau (CAB): 1. LPA observed room #1 only has one resident's furniture. However, room #1 was identified as a shared room. 2. LPA observed room #6 has window coverings that does not provide privacy for resident. 3. LPA observed cleaning supply cabinet does not have a lock and knives were unlocked in the kitchen cabinet. (Continue on LIC809C...) 4. LPA observed the backyard had overgrown grass/weeds. Licensee(s)/Applicant will submit proof of corrections to CCLD on/before 3/13/2026. Exit interview conducted and a copy of this report provided.
