Pre-licensing visit·March 13, 2026

On 3/13/2026 at 8:30AM, Licensing Program Analyst (LPA) G. Luk conducted a Pre-licensing Inspection to verify corrections were made. LPA met with Licensee/Applicant, Saul Cantolino. LPA toured facility including but not limited to resident's bedrooms, bathrooms, living room, kitchen, and outdoor area. LPA observed room #6 has window coverings that covers the entire window for privacy. Room #1 has two sets of furniture including beds, night stands, lamps, chest of drawers, and chairs for each resident. Facility has a locked cabinet in the garage for cleaning supplies and a lockbox to store scissors/knives. LPA observed both cabinet and lockbox was locked during inspection. LPA observed the grass/weeds in the backyard has been trimmed down. No issues noted during inspection. LPA observed that facility is ready to be licensed. This report will be submitted to the Centralized Application Bureau (CAB) and a final review of the application will be conducted. This facility is not yet licensed and is subject to final approval by CAB. Additional requirements may still be required. Exit interview conducted and a copy of this report provided.
