What the inspector wrote

During the investigation process, the administrator and family members were interviewed. The residents were observed; however, were not interviewed due to their cognitive impairment. In addition, LPA Gurriere toured the apartments of the memory care residents. The allegation is that “Staff are retaliating against residents for making complaints.” It is noted that residents did not make complaints. It was reported that the family members voiced concerns that the administrator removed video cameras from two resident rooms. The administrator reported that she did take cameras out of resident rooms, as the facility corporation did not have a policy in place when cameras were installed. The administrator advised that she removed the cameras as a directive from the corporation. On 04/15/24 a complaint investigation was conducted and addressed the issues surrounding cameras allowed in resident rooms; allegation was Substantiated. See report for additional details. It has been determined that staff did not retaliate against residents for making complaints. Although the above allegation mentioned may have happened, or is valid, there is not a preponderance of evidence to prove that the alleged violation occurred, and the above findings are Unsubstantiated .

    Infection Control Requirements - A licensee shall ensure that infection control practices are maintained as follows: All staff and volunteers shall perform hand hygiene. Hand hygiene shall be conducted before and after assisting with medications. This requirement was not met as evidenced by: Based on video feed observed by LPA Gurriere, the licensee did not ensure that infection control and hand hygiene were used when administering medication. This poses an immediate risk to residents in care.
