05/09/2024 01:00 PM Licensing Program Analyst (LPA) Rebecca Knight, made an unannounced visit to the facility and met with Executive Director Sonya Gonzalez. The purpose of this visit was to deliver the results of a complaint investigation. It was reported that residents were forced to stay in their rooms during a recent outbreak of Covid 19. During the course of the investigation LPA interviewed the Executive Director and 7 residents. LPA reviewed the following documents: Staff roster with telephone numbers, resident roster. Six of seven residents interviewed stated they were not forced to stay in their rooms during a recent Covid outbreak. This agency has investigated the complaint alleging staff are confining residents in their rooms. We have found the complaint was UNFOUNDED, meaning that the allegation is false, could not have happened, and/or is without a reasonable basis. An exit interview was conducted. A copy of the report was provided to Executive Director Sonya Gonzales.
