09/04/2025 Licensing Program Analyst Rebecca Knight conducted an unannounced case management visit and met with Executive Director April Kale . Today’s visit is regarding an incident report that was received by licensing on 07/11/2025 regarding an incident that occurred on 07/06/2025. It was reported that on 07/06/2025 Resident 1 (R1) was having a manic episode and was not acting like themself. R1 was urinating in a cup and then was drinking it. EMS was called and R1 was transported to Oroville Hospital and admitted for observation. During the investigation it was learned that this behavior is not normal for R1. R1 is currently at Enloe Hospital and has been accepted at a mental health facility to have their medications stablilized and to be provided counseling. Once R1 is back at baseline they will be returning to live at the facility. No deficiencies were cited as a result of today’s visit. Exit interview conducted and a copy of the report was provided to with Executive Director April Kale.
