09/25/2025 11:00 AM Licensing Program Analyst (LPA) Rebecca Knight, made an unannounced visit to the facility and met with Executive Director April Kale. The purpose of this visit was to obtain current status on the items that LPA issued a physical plant citation for 08/28/2025. LPA and ED toured the facility together and LPA made the following observations: The facility wifi / internet had not been functioning since 08/11/2025. During today's visit LPA learned that on 09/04/2025 the new internet vendor completed installation of temporary routers throughout the building and hot spots have not been used since that date. The installation of the permanent equipment will be completed at a future date and ED has agreed to notify LPA once the date has been confirmed. The carpet in the memory care unit and assisted living common areas is stained and dirty. LPA observed the carpets in the memory care unit and assisted living common areas to still have significant staining. ED confirmed that the carpets have been cleaned two times since 08/28/2025. The carpets are being professionally cleaned every two weeks. ED has agreed to send LPA the receipts for all carpet cleanings and will continue to send LPA receipts for all carpet cleanings moving forward. The licensee has requested bids for the replacement of all carpet in the common areas of the memory care and assisted living sections of the building and will send LPA these bids once received. Continued on LIC809-C The fence that surrounds the garden in the memory care unit is in disrepair. LPA observed that the facility has replaced / repaired the areas of the fence that have broken fence boards. The licensee has requested bids to replace the fence and address any area of erosion of the ground under the fence. ED has agreed to send those bids to LPA once received. LPA will continue to monitor. No deficiencies were issued as a result of today's visit. Exit interview was conducted and a copy of the report was provided to Administrator April Kale.
