11/25/2025 10:40 AM Licensing Program Analyst (LPA) Rebecca Knight, made an unannounced visit to the facility and met with Executive Director (ED) April Kale. The purpose of this visit was to obtain current status on the facility's telephone system. On 11/12/25 ED reported to LPA that the facility telephone system was not allowing incoming calls. ED reported that they were working with the telephone vendor to resolve the issue. During the outage facility staff contacted resident families and instructed them to call the memory care number of 530-534-8586 which was still operational. The hospital and physicians had the direct mobile phone number for the Resident Care Coordinator if they needed to make contact. During today's visit ED stated that Comcast had restored service on 11/24/2025. LPA verified that the main telephone number was accepting incoming calls during the visit by dialing the number and confirming that facility staff answered. No deficiencies were issued as a result of today's visit. Exit interview was conducted and a copy of the report was provided to Administrator April Kale.
