What the inspector wrote

Staff do not keep the facility free of pests – SUBSTANTIATED During a tour of the facility LPA observed cock roaches in one resident room, cockroach traps in one resident room and one office. The facility currently has pest control coming in once per week to treat known problem areas but the facility is still experiencing issues with cockroaches. A more aggressive treatment plan to include deep cleaning of the problem areas is required in order to mitigate the issue. Based on interviews and evidence obtained during the investigation, the preponderance of evidence standard has been met, therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22), is being cited on the attached LIC9099D. Appeal rights were provided. Exit interview conducted and a copy of the report was provided to Executive Director April Kale.
