Thirty (30) days written notice....the licensee and the person who performs the reappraisal believe that the facility is not appropriate for the resident. This requirement is not met as evidence by: Based on documentation review, the notice served to R1 did not include a valid reason for eviction, which poses a potential health, safety, and personal rights violation to the residents in care.
