What the inspector wrote

On July 28, 2025, Licensing Program Analyst (LPA) Kayla Adkison, arrived at the facility unannounced to conduct a 1-Year Required Annual Inspection. LPA met with Facility Administrators, Anthony Faulkner and Maria Boulangger Falla, and explained the purpose of the visit. LPA and Administrators toured facility together to ensure health and safety of residents in care. LPA observed the facility to be clean, in good repair and odor-free. In the areas toured no immediate health, safety, or personal rights violations were observed. Areas toured include but are not limited to: common areas, resident bedrooms, kitchen, common restrooms, storage area, and exterior courtyards. Facility has a 2-day perishable and a 7-day non-perishable amount of food. LPA observed all sharps and cleaning supplies to be kept locked and inaccessible to residents in care. LPA observed the medications to be kept in a locked room and inaccessible to residents. LPA observed fire extinguishers, fire detectors, and carbon monoxide detectors which were last inspected in July 2025. LPA observed the first aid kit to be complete and ready for emergency use. The facility was unable to provide documentation that the emergency disaster drills has been conducted since December, 2024. LPA reviewed a total of three (3) residents' files and three (3) staff files. All staff files contained the required documentation. Two of three resident files were missing record of a tuberculosis exam. Deficiencies cited from Title 22 Regulations and or the California Health and Safety Code. An exit interview was conducted and a copy of this report, LIC 809-D, and Appeal Rights were discussed and provided.
