What the inspector wrote

On 5/15/2024 at 9:00 AM, Licensing Program Analyst (LPA) A Gomez arrived unannounced to conduct a complaint investigation and deliver findings in regard to the allegations above. LPA met with Executive Director (ED), Julie Mammad and explained the purpose of the visit. During the visit LPA interviewed S1. S1 informed LPA that a visitor had been harrassing them which is why that visitor was asked not to visit temporarily. LPA obtained documentation of the ED stating that a visitors visitations rights were temporarily suspended. Based on LPAs observations and interviews which were conducted and record reviews, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. California Code of Regulations (Title 22, Division 6, Chapter 8), are being cited on the attached LIC 9099D.
