On March 26, 2025, at 1:00 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to investigate the allegation above. Upon entry into the facility, the LPA identified himself and stated the purpose of the visit to Executive Director (ED) Monique Bindra. The complaint alleges that staff does not ensure residents' rooms are clean. The LPA interviewed the ED. She stated that they had an unexpected staff shortage. Staff were moved around and staff from other facilities were brought in to ensure rooms were deep cleaned at least once a week. The data collected does not confirm the allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove it; therefore, the allegation is UNSUBSTANTIATED. Exit interview conducted and a copy of this report was provided.
