What the inspector wrote

On 10/13/2025 at 1:00PM, Licensing Program Analysts (LPAs) Andrew Christy and James Sampair arrived unannounced to conduct the Required Annual Inspection. Upon entry, LPA stated the purpose of the visit to Staff Members Alena Ilagan and Asterio Quiblat. Manager Vicente Deperalta arrived at approximately 2:00 PM. The LPAs inspected the interior and exterior of the facility. The inspection of the physical plant included the kitchen, dining area, restrooms, community living spaces, resident rooms, storage areas, garage, and the grounds of the facility. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. The hot water temperature in the resident's bathroom was measured to 114.8 degrees Fahrenheit and the living room temperature was 71.4 degrees Fahrenheit. The fire extinguisher was last serviced on 09/16/2024. The carbon monoxide and smoke detectors were fully operational. The LPAs observed required postings in the facility, including the Residential Care Facility for the Elderly Complaint Poster, Ombudsman and Personal Rights posters, and the Theft and Loss Policy. LPAs reviewed the facility records, records of 5 residents, and records of 5 staff members. The Emergency Disaster Plan was last reviewed 10/13/2025. Quarterly emergency drills were last conducted 09/06/2025. Continued on LIC809C..... Continued from LIC809..... The following deficiencies were cited during the inspection: At 1:20PM, LPAs found an unlocked cabinet in the garage with cleaning chemicals like bleach and Comet. As well, there were screws in the drawers. At 1:45PM, there was a bathroom discovered to have an unlocked cabinet under the sink with various cleaning chemicals, as well as more screws in the drawers. At 1:05PM, the fire extinguisher was found to be expired. Deficiencies are cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties. Exit interview conducted. A copy of this report, along with appeal rights, was provided to the administrator.

This requirement is not met as evidenced by: Based on record review, licensee did not comply with the section cited above. Staff S1 was not fingerprint cleared, which poses an immediate health and safety risk to the persons in care.

Staff S2 and S3's fingerprint clearance had not been transferred to this facility, which poses an immediate health and safety risk to the persons in care.
