What the inspector wrote

On 10/13/2025, at 1:00 PM, Licensing Program Analysts James Sampair and Andrew Christy arrived unannounced for this Case Management - Deficiencies visit. Upon entry, the LPAs explained the purpose of the visit to Caregivers Elena Ilagan and Asterio Quiblat, who were later joined by Managers Lucas Argueza and Vincente De Peralta. The Licensee was cited for fingerprinting violations for 3 staff members. 2 Type-A citations were issued during this inspection (for details refer to LIC 809-D). Deficiencies are cited from Title 22 California Code of Regulations. Failure to submit proof of correction may result in additional civil penalty. Exit interview conducted, a copy of the Appeal Rights, and this report provided.

Staff S1 was not fingerprint cleared, which poses an immediate health and safety risk to the persons in care.
