What the inspector wrote

On 10/1/2021, during the course of another visit, Licensing Program Analyst ( LPA ) L. Ibo and Regional Center of East Bay QA Synthia Hakola conducted a joint case management visit . LPA interviewed resident and staff. Based on the interview, one staff (S2) was present at the facility on 10/1/2021 and worked early morning shift (breakfast) . Based on the interview with residents S2 has been working at the facility for about a month now. LPA conducted records review using guardian system and S2 is not finger print cleared from DOJ & FBI. $500.00 Civil penalty was assessed. Deficiencies are cited from Title 22 California Code of Regulations (see 809D). Failure to submit proof of corrections by plan of correction due dates, and any repeat violations within 12-month period may result in civil penalties. Exit interview conducted, appeal rights and copy of this report provided.
