What the inspector wrote

On 11/19/2025 at 9:00 AM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Administrator, Adelaida Manalang, and explained the purpose of the visit. Administrator certificate is current. LPA toured facility inside and out including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 5 total bedrooms which are occupied by the residents. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature is maintained at 72 degrees Fahrenheit. LPA observed lighting in all rooms is adequate for the comfort and safety of the residents. Residents’ bathrooms are equipped with grab bars and non-skid shower pans. There is a minimum of one week supply of nonperishable and 2-day of perishable foods. Smoke detectors and carbon monoxide detectors were in operation during visit. Fire extinguisher was last serviced on 05/19/2025. First aid kit was observed to be complete. Emergency disaster drill was last conducted on 08/18/2025. At 10:03 AM, LPA reviewed 5 residents records. At 10:25 AM, LPA reviewed 4 staff records and 4 of 4 have current first aid training and associated with the facility. At 11:47 AM, LPA reviewed a sample of resident’s medications. Updated copies of the following documents were requested for facility file and are to be submitted to CCL by 12/03/2025 LIC 610E Emergency Disaster Plan THE FOLLOWING DEFICIENCIES WERE OBSERVED DURING VISIT: At 9:27 AM, LPA observed the hot water temperature measured at 132.5 degrees Fahrenheit. At 9:43 AM, LPA observed a key lock on the only emergency side gate. At 9:52 AM, LPA observed Multivitamin Gummies, Tylenol, Triamcinolone Acetonide, etc. and all-purpose cleaning wipes At 11:48 AM, record review revealed that all residents have half bed rails without a doctor’s order. At 11:59 AM, record review revealed that S2 and S3 are missing LIC503, Health Screening, and S3 is missing TB test. At 12:00 PM, record review revealed that all staff do not have staff training on file. The Facility was cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiencies by POC date may result in additional Civil Penalties. Exit interview conducted with Administrator. Appeal Rights and a copy of this report provided.
