What the inspector wrote

On 01/22/2026 at 12:10 PM, Licensing Program Analysts (LPAs) P.Manalo and A. Christy arrived unannounced to conduct a Case Management inspection of Qcare Residential Facility II. LPAs met with staff, John Ventura, to ensure the facility is in compliance with applicable statues and regulations. Assistant Administrator, Gladys Cuevas Villa, came shortly after. During the visit, LPAs conducted interviews with residents and staff. Based on interviews with the residents, 4 of 6 residents stated that they are happy and like the food at the facility. Interviews with 2 of 2 staff members are comfortable with the living situation at the facility. 4 of 4 staff stated that there are no issues with receiving their paychecks on time and all stated that there is coverage when staff needs to go on break or on their days off. LPAs observed the California Minimum Wage and Industrial Welfare Commission Wage Orders poster but is not the updated year. LPAs are requesting for staff to send updated LIC500 by 01/23/2026. LPAs conducted a tour of the physical plant and observed the following deficiencies: At 12:30 PM, LPAs observed the hot water temperature measured at 127.6 degrees Fahrenheit. At 12:37 PM, LPAs observed unlock scissors, mallet, and multi-tool keychain. Continue to LIC809-C... Continue from LIC809.. At 12:45 PM, LPAs observed a bolt locking the side gate. The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiencies by POC date may result in additional Civil Penalties. Therefore, the facility appears to be not in compliance. An exit interview was conducted and a copy of the report was provided.

    (2)...Hot water temperature controls shall be maintained to automatically regulate the temperature of hot water used by residents to attain a temperature of not less than 105 degree F (41 degree C) and not more than 120 degree F (49 degree C).This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above by having the hot water temperature measured at 127.6 degrees Fahrenehit which poses an immediate safety risk to persons in care.

    Ensure hazardous items are locked and not unattended

    (a) Except as specified in subsection (b), the licensee shall ensure that disinfectants, cleaning solutions, poisonous substances, knives, matches, tools, sharp objects...which could pose a danger to residents are in locked storage and are not left unattended if outside the locked storage.This requirement is not met as evidenced by: Based on observations, the licensee did not comply with the section cited above by having scissors, mallet, and multi-tool keychain unlocked which poses an immediate safety risk to persons in care.

  • Fire approval and staff access to unlock systems

    (f) ... (2) The licensee shall ensure that the fire clearance includes approval of locked exterior doors or perimeter fence gates and that facility staff on all shifts have access to, and know how to use, equipment needed to unlock exterior doors or perimeter fence gates.This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above in having a bolt locking the side gate latch in the backyard which poses an immediate health and safety to persons in care.
