On 10/21/2021 at 3:15PM, Licensing Program Analysts (LPAs) L. Hall and J. Clancy-Czuleger arrived unannounced to conduct an Infection Control Inspection. LPAs met with Licensee, Angelito Marable and explained the purpose of the visit. Upon entry, LPAs temperature was checked, and LPAs observed screening station that contained hand sanitizer and log. LPAs toured facility including but not limited to common areas, bathrooms, bedrooms, and kitchen. LPAs observed cough etiquette and physical distancing posted in the common areas. All hand washing stations were equipped with soap, paper towel, garbage with a lid, and hand washing posters were posted. During record review, LPAs observed visitors log and temperature log for both residents and staff. LPAs observed facility has a copy of Mitigation Plan on file. LPAs observed PPE, food, and paper supplies are sufficient. -On 10/21/2021 at 3:35PM, LPAs observed that staff 2 (S2) was not associated to facility. The deficiency were observed (see LIC 809D) and cited from the California Code of Regulations. Failure to correct deficiency by POC date may result in additional Civil Penalties. Exit interview conducted. A copy of this report and appeal rights provided.
