What the inspector wrote

On 7/9/2021 at 11:30AM, Licensing Program Analysts (LPAs), G. Luk and C. Fowler arrived unannounced to conduct a case management visit. LPAs met with Facility Supervisor, Marilou Ladaban and Administrator, Elizabeth Cortes. While conducting a complaint investigation, LPAs observed the following deficiency: Facility had an administrator change in March 2021, but did not notify CCLD in writing within 30 days. Facility notified LPA P. Singh in-person on May 2021. The deficiency was observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct deficiency may result in civil penalties. Exit interview conducted. A copy of this report and appeal rights was provided.
