Inspector’s narrative

What the inspector wrote

On 05/21/2020, the rehabilitation center called the facility to arrange for R1’s return to the facility. The staff who answered the call told the rehab center to call back the next day because facility will not be accepting R1 back to the facility. The rehab center was not informed of R1’s relationship with the department of Public Health, Placement Authorization and Utilization management, Transitions-Placement Team until 4 days later. The preponderance of evidence has been met. Therefore, the allegation is substantiated. Deficiency is cited per Title 22 California Code of Regulations and listed on LIC9099D. Failure to submit proof of corrections (POC) by plan of correction due dates and/or any repeat deficiencies within a 12-month period may result in civil penalties. Exit interview conducted. Appeal Rights and a copy of this report provided via email.
