On 04/07/2026 at 9:00 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct this Case Management visit to verify status of Licensee's property and business ownership. Upon entry, the LPA stated the purpose of the visit to Caregiver Alisha Pina. During the visit, the LPA conducted phone interviews of Licensee Kamilla Distefano, Staff S1, and Staff S2. The phone interviews were conducted separately and in short succession. Each of the three interviewees made similar statements that the Licensee does not have a lease agreement nor sold controlling interest of A Place for Seniors, LLC to S2. No citations issued during visit. Exit interview conducted and a copy of this report provided.
