What the inspector wrote

On 11/12/2025 at 9:45 AM, Licensing Program Analyst (LPA) Carol Fowler arrived unannounced to conduct the Required Annual Inspection. Upon entry, LPA stated the purpose of the visit to Marilyn Judan, Administrator. LPA toured the facility including but not limited to bedrooms, bathrooms, dining area, activity rooms, kitchen, common areas, and outdoor area. Centrally stored medications were locked in medication cabinet located in the kitchen. Smoke detectors and carbon monoxide detectors were observed in working condition. Fire extinguishers were observed to be full and last serviced on 10/18/2025. Temperature in the facility was measured at 72.0 degrees Fahrenheit. Water temperature is 113.7 degrees Fahrenheit. The LPA observed required postings in the facility, including the Complaint Poster, Ombudsman and Personal Rights posters, and the Theft and Loss Policy. An administrator is on site more than the minimum of 20 hours a week to oversee the proper business operations. Grab bars for each toilet and shower were installed. Non-skid mats were observed. There were adequate lights in each room. Resident rooms were observed to be cleaned and fully furnished. . One week of nonperishable and 2 days of perishable food and water supplies were available. Continue on LIC 809C CONTINUE FROM LIC 809 LPA observed the following deficiencies: · At 1:39pm, LPA observed R1 with a full bed rail and not on hospice. · At 1:44pm, LPA observed room #4 closet door jam with dust. · At 1:47pm, LPA observed front door window with dust and spider webs. At 2:00pm, LPA observed artificial turf rolled up in front of the gate. At 2:05pm, LPA observed 2 2x4's outside of the open porch. At 2:20pm, LPA observed the garage door unlocked with the key hanging from the knob, the garage contains chemicals, tools and other potential items. At 2:28pm, LPA observed the kitchen cabinet and microwave has grease and dirt on them. At 2:40pm, LPA observed foods not properly stored in the refrigerator and freezer. LPA requested the following documents to be submitted to CCLD by 11/20/2025. · LIC 308 Designation of Administrative Responsibility · LIC 309 Administrative Organization · LIC 500 Personnel Report · LIC 610E Emergency Disaster Plan (9 pages) · Liability Insurance The deficiencies were observed (see LIC809D) and cited from the California Code of Regulation, Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties. Exit interview conducted and a copy of this report and appeals right provided.
