What the inspector wrote

On 06/18/2025 at 10:00AM, Licensing Program Analyst (LPA) T. Syess-Gibson conducted an unannounced 1-Year Required inspection. LPA met with Caregiver, Linda Dela Cruz, and explained the purpose of the visit. Linda contacted the administrator via telephone. Administrator, Garry Malabato arrived at 11:03AM, LPA explained the purpose of visit. Administrator currently holds a certificate (#7007516740) expires 02/10/2027. The facility’s fire clearance was approved for five (5) non ambulatory and one (1) bedridden resident. LPA toured the facility with Administrator including but not limited to bedrooms, bathrooms, kitchen, common area and back yard. The facility consists of four (4) bedrooms and two (2) bathrooms. All outdoor and indoor passageways are kept free of obstruction. LPA did not observe any bodies of water. A comfortable temperature is maintained at 71 degrees Fahrenheit LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 123.0 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and nonskid mats. There is a minimum of 7-day supply of non-perishable and 2-day of perishable foods. Continue on LIC809C Continued from LIC809 Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last purchased on 10/11/2023. Emergency Disaster Plan was last posted on 07/12/2024. First aid kit was observed to be complete. LPA reviewed all three (3) resident records, three (3) staff records and they were current and complete. The following forms to be updated and submitted to CCLD by 06/25/2025: LIC 308 Designation of Administrative Responsibility LIC 309 Administrative Organization LIC 500 Personnel Report LIC 610E Emergency Disaster Plan Continue on LIC809C Continue from LIC809C LPA observed the following deficiencies: At 10:53AM, LPA observed scissors in an unlocked drawer in residents’ shared bathroom At 10:56AM, LPA observed resident in room# 3 in a hospital bed with half ½ rail, during file review, LPA observed there were no orders approving medical bed with ½ rail from a medical professional The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct deficiencies may result in Civil Penalties. Exit interview conducted. Appeal Rights and a copy of this report provided to Gary Malabato.
