What the inspector wrote

On 8/12/2021 at 1:50PM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conduct a Case Management inspection. LPA met with Interim Executive Director, Tracy Freudendahl and informed her the reason for visit. While LPA was at the facility conducting another visit, the following deficiency was observed. During visit, LPA observed that S1 was not associated to the facility. LPA verified that S1 was fingerprint cleared, but not associated to the facility. The deficiency was observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct the deficiencies may result in civil penalties. Exit interview conducted. A copy of this report and appeal rights were provided.
